HOME BASED SERVICES

 

Program Mission Statement:

Our goal is to preserve the natural family unit and to foster a safe and healthy environment within the family setting.  We seek to promote family growth and development to enable the child to remain with family and extended family members whenever possible.  Within the context of community-based treatment, the child will have the opportunity to remain in the home, school, and be linked to resources in the local community.

Program Description:

Home-based services are designed to provide intensive individual and family therapy within the home and community setting with children and families having multiple service needs.  This modality provides access to a continuum of mental health services.  Services are provided in the home, school, or location within the community.  Modalities may include any or all of the following:

-  Individual, family, and group therapy

-  Crisis Intervention

-  Service Coordination

-  Other Mental Health Services

-  Psycho-education/Skillbuilding

 

Goals / Indicators:

-  Satisfaction:  Consumers will report 85% satisfaction with services received. 

-  Efficiency:  Location of service delivery will occur outside the clinic 60% of the time.

-  Effectiveness:  80% of the time, consumers and their families will report significant improvement as a result of Home Based Services.  Current families in service will give their report to Quality Personnel who contact them during any given quarter.
