Meridian Heights AFC
Mission Statement:
To promote community living for individuals with mental illness and / or Developmental Disabilities.
Program Description:
Meridian Heights will serve individuals with mental illness and / or Developmental Disabilities. Services provided within the home will include assistance with activities of daily living, meal preparation, assistance with administering medication, assistance with budgeting, and assistance with medical appointments. Each resident will be encouraged to participate in community outings as part of the ongoing effort to integrate individuals into their community. Services may include skill building and / or community supported living.
Program Goals:
1. Medicaid Verification - 95%compliance
2. Report - 90% or higher consumer satisfaction
3. Medication errors - 100% no errors which result in adverse reactions.
