outpatient Services Purpose: To provide meaningful behavioral health services that restore individuals and families to functioning in family, social, educational and work place roles. Philosophy: Outpatient counseling services are short-term services which strive to be educational, motivational, and supportive in nature, assisting clients in enhancing and utilizing their own strengths, skills and abilities to recover from, or cope with mental illness Services: All persons who request counseling services from Hiawatha Behavioral Health will be screened for appropriateness for outpatient care. Persons meeting eligibility criteria will be offered an assessment within two weeks of the request for service. HBH strives for efficient access to service; we do not have waiting lists. Persons who are found to be ineligible for HBH services, will be referred to services which meet their needs. There is a 3 session admission process which helps establish the Person Centered Planning Process (PCP). A preliminary plan of care is signed in the first session which covers outpatient services for up to 2 months and begins the preplanning part of the PCP process. The assessment may be started during the first session but is often concluded in the second session. Usually, within the third session, the formal Person Centered Plan is completed. Cases that can be concluded within 4 outpatient sessions will be done so under the preliminary PCP. At HBH, the plan for services and supports is designed by the person served through the PCP process. Outcomes are designed, through a process of negotiation, to fulfill the needs, hopes, dreams, desires, expectations and goals of the person served. The plan utilizes the strengths, abilities and family/social/community supports of the person served. Outpatient services consist of counseling with consumers in individual, group, or family sessions. Outpatient may be provided in conjunction with medication therapy, case-management and vocational services. Most of the counseling is conducted at the three office sites of Hiawatha Behavioral Health. For persons who are unable to attend outpatient sessions at the clinic, we will provide services in the home, local jail, hospital, or nursing home. |
