Supported Community Living
Mission Statement:
The Supported Community Living Program's Mission is to promote independence and enhance the quality of life for people with developmental disabilities.
Description of Services:
Supported Community Living Technicians will provide services in the following areas:
1. Shopping
2. Meal Preparation
3. Cleaning
4. Attending routine medical appointments
5. Bathing / Grooming
6. Budgeting
7. Assistance with medications
8. Laundry
9. Use of Community resources
Program Goals:
1. Effectiveness:
95% of Supportive Community Living consumers will successfully maintain their independent living setting.
2. Efficiency:
Within 10 business days of authorization from Placement Review Commitee, Supported Community Living Supervisor will determine the start of services.
3. Consumer Satisfaction:
Consumer Satisfaction of Supportive Community Living Program will receive a 92% or better rating. Surveys will be done twice yearly.
