Program Description:

Coordinating community and natural resources through Person Centered Planning to provide goal orientated treatment plans and self-advocacy. A holistic approach to independence utilizing community agencies and natural supports, the role of supports coordination is to assist with self-advocacy, self-determination and facilitated choices as directed by the consumer to accomplish community integration.

Goals / Indicators:

1. Delivered services will have backup documentation in files - 95% compliance. (Efficiency)

2. Consumers / Guardians will report an increase or maintenance of functioning - 70% or higher (Effectiveness)

3. Consumer / Guardians will report a high level of satisfaction - 85% or higher. (Satisfaction)
