Take five, ADULT DAY PROGRAM

 

Mission Statement:  

To offer families support services they need in order to live in their community with dignity and respect.

Program Description:  TAKE FIVE Adult Day Services is a day program that serves persons with some form of dementia.  Hiawatha Behavioral Health TAKE FIVE Day Program operated three days a week, Tuesday, Wednesday, and Thursday for six-and-a-half hours a day.  TAKE FIVE is a social activities program that provides a morning snack, noon meal and an afternoon snack.  The caregiver receives respite from the tasks of care giving and the person affected with the memory loss receives stimulation by participating in a variety of social and physical activities.  There is a staff / volunteer to consumer ratio of one to three.  This ratio may vary depending on the consumer's functional abilities.

Program Goals: 

1.  Required paperwork from programs will be completed and received within 30 days - 80% of the time.  (Efficiency)

2.  Report 92% or higher Consumer Satisfaction. (Satisfaction)

3.  The program's overall occupancy will be 80% or higher(Efficiency)
